Alan Strauss est un psychothérapeute séquestré par un patient qui se révèle être un tueur en série. Sam fait une demande de traitement inhabituelle à Alan : réfréner ses pulsions homicides. Pour survivre, Alan doit analyser l'esprit torturé d'Adam et l'empêcher de tuer à nouveau… mais Sam refuse d'aborder des sujets cruciaux. Tout au long de son emprisonnement, Alan découvre non seulement combien les pulsions de Sam sont ancrées dans son subconscient, mais aussi tout le travail qui l'attend pour combler le fossé qui s'est creusé au sein de sa propre famille. Alors que le temps presse, Alan lutte désespérément pour arrêter Sam avant de devenir lui-même complice de ses meurtres ou pire, une cible.
